Abstract

Flexible bronchoscopy requires sedation that maintains spontaneous breathing and ensures patient tolerance. Objective: To analyze the efficacy and safety of dexmedetomidine compared with propofol for sedation during flexible bronchoscopy in adults. Method: Qualitative systematic review following PRISMA 2020 in PubMed/MEDLINE (2012–2026), with content analysis. Results: Ten studies were included: six randomized trials and four systematic reviews with meta-analysis. Dexmedetomidine was associated with less hypoxemia (odds ratios between 0.20 and 0.40 versus propofol) and with less cough in some trials, but it caused more bradycardia, slower recovery and, depending on the protocol, more hypotension and more rescue boluses. Conclusions: Dexmedetomidine is a useful alternative when respiratory protection is the priority, whereas propofol offers faster onset and recovery; the choice should be individualized according to the patient’s cardiorespiratory risk and the expected duration of the procedure.
