Introduction: Family caregivers of patients with cancer face physical, emotional, social, and economic demands that may negatively affect their quality of life. Objective: To analyze the scientific evidence on factors associated with the quality of life of family caregivers of patients with cancer. Method: A systematic review was conducted following PRISMA 2020 guidelines, with a protocol registered in PROSPERO: CRD420261378686. The search was performed in Cochrane Library, PubMed/MEDLINE, Scopus, Web of Science, LILACS/BVS, SciELO, and Redalyc. Original studies published between 2021 and 2026 were included. Results: A total of 47 studies were analyzed. Factors negatively associated with quality of life included anxiety, depression, uncertainty, emotional distress, caregiver burden, financial difficulties, and low social support. Protective factors included resilience, family and social support, spirituality, self-efficacy, caregiver preparedness, and positive coping strategies. Conclusions: Comprehensive interventions focused on the patient-caregiver dyad and family context are required.
