Factors contributing to infection at the insertion site of central venous catheters

Objectives: To interpret closed-circuit secretion aspiration in the prevention of mechanical ventilator-associated pneumonia in intensive care unit. Method: Retrospective observational study of exploratory level through systematic literature review in Medline/PubMed, WOS, Scielo, Scopus and Google Scholar databases, period 2017-2022. Ten primary studies and 15 secondary studies were selected according to established inclusion and exclusion criteria. Results: Closed suction aspiration may decrease ventilator-associated pneumonia according to some studies, although this was not considered in all reviewed studies. The closed system is preferred for presenting fewer complications such as environmental contamination and cross-infections, has higher cost but greater effectiveness maintaining stable intracranial pressure and lung volume. Conclusions: Closed-circuit aspiration is not consistently associated with lower incidence of ventilator-associated pneumonia, but may prevent pneumonia in specific cases.
