Objective: To analyze the ketogenic diet in patients with refractory epilepsy. Case report. Method: It was based on a quantitative method, applying a documentary-bibliographic typology for the analysis. Supported by statistical and descriptive analysis. Results: It was evidenced that the implementation of the ketogenic diet (KD) in the patient produced a drastic and sustained decrease in the frequency of epileptic seizures. In conclusion: The implementation of the ketogenic diet in a pediatric patient with refractory epilepsy showed an immediate reduction, even after progressive suspension of the dietary intervention. The favorable response in a context of pharmacological refractoriness reinforces its usefulness as a complementary therapeutic strategy in cases that do not meet surgical criteria
